Clinical Effectiveness Group
We support primary care in east London and are based in the Centre for Primary Care and Public Health within the Medical School at Queen Mary University of London. Our clinician led service provides tools for GP practices such as clinical templates, in-practice facilitation and health informatics for commissioning and clinical management. We have been supporting primary care for over 20 years. Our multidisciplinary team includes GP’s, practice facilitators, data analysts, and researchers.
Evidence-based Clinical Guidelines
CEG translates new initiatives and research on quality and clinical excellence into local primary care through evidence-based clinical guidelines and care pathways. CEG has also helped develop and implement a number of influential clinical risk scores, including QRisk and QDScore. Our clinical guidelines and care pathways are available to download here.
Software Tools to Improve Clinical Recording
We provide a wide range of software tools for EMIS web practices. SMART TEMPLATES. Our clinical templates are deployed in EMIS library and are available nationally. We have a number of SMART Templates incorporating visibility rules – e.g Long Term Condition Template can be used for patients with multi morbidities.
We have expertise in producing templates for other clinical systems e.g Vision. Template guides detailing content of each template can be downloaded here.
SMART DATA we have a suite of EMIS web searches for practices to enable effective delivery of clinical care, optimise income through QOF and extended primary care services and support call and recall of patients. For example, we provide monthly recall searches for NHS Health Checks, diabetes retinopathy screening and high CVD risk. Our suite of Prevalence searches can be downloaded here . We produce search manuals which detail all the searches that are available for practices which can be downloaded here.
SMART CLINICAL MANAGEMENT TOOLS We produce a number of tools to support clinical decision making. They provide key diagnostic and treatment indicators in a simple format so that better optimal patient care and QOF points are achieved.
Available for use:
The APL Tool -Atrial Fibrillation: – this tool rapidly identifies high risk patients with AF who are not taking anticoagulants. It enables priority patient list to contact for reviews. It automatically calculates a HASbled score.
The Heart Failure Tool- this tool displays the total number of patients with heart failure and LVSD. Only people with LVSD coded count towards QOF targets. The tool shows a number of key indicators some of which are in QOF and some are wider evidence based indicators.
The CKD Tool: this tool flags up patients whose kidney function may be declining. In east London we have been instrumental in setting up a community renal clinic and this tool identifies patients who may benefit from referral to the community renal clinic.
Contact us directly for further details about these SMART Tools.
Facilitating Change, Facilitating Improvement
The facilitation work stream of the Clinical Effectiveness Group provides a proactive approach to assist NHS organisations to meet targets and to enable them to work understand their patients/population. Working closely with practices we assist in the improvement of data quality, reduction of inequalities and development of clinical quality.
Through facilitation we can help with:
- Understanding clinical coding
- Using clinical system templates
- Implementing clinical guidelines
- Constructing EMIS web searches
- Understanding data sets, including dashboard reports
Workshops and Online Training Services
CEG Facilitators and clinicians provide educational sessions including multidisciplinary workshops, Practice Learning Time sessions and IT training clinics. These educational events last from one day to half day sessions and are aimed at all practice staff. We offer one day courses on contemporary clinical topics which involve teaching from our secondary care colleagues.
We develop training materials for practices including EMIS web instructions and a suite of on line training videos. All available to view and download.
Check our events page for details of upcoming workshops.
CEG’s team of data analysts produce a wide range of data reports and dashboards to ensure a better understanding of clinical activity, patient populations and developing health trends. Our dashboards are used by GP Practices, GP Federations, healthcare networks/clusters, CCGs, and Local Authority Public Health departments to improve clinical services and local health quality. We provide data results for NHS England and other health bodies, to enable insights and the development of national healthcare. We also produce tailored one-off reports for specific service planning and research projects.
- Local Service Dashboards: Local service monitoring and payment calculation for practices, networks/clusters and CCGs/Public Health.
- Activity and Management Audits: Overviews of disease management and clinical activity:
- GP prescribing improvements.
- Flu vaccination uptake
- Self-Monitoring of Blood Glucose
- Anti-Coagulation in Atrial Fibrillation.
- Joint Strategic Needs Assessment: Prevalence of selected diseases with breakdowns by age, gender, ethnicity, deprivation and vulnerability.
- CEG Annual Reports: Analysis of borough level health trends for age, gender, ethnicity, deprivation and vulnerability.
- National Health Returns: Data provided for NHS England, QIPP Programme, Public Health England and Governmental data requirements.
The CEG team leads research in the effective delivery of primary care in an ethnically diverse, inner city environment. The findings of this research are directly translated back into health practice through the development of clinical guidelines, risk scores and software tools.
